Birth Certificates - Applications

BIRTH CERTIFICATES
Individuals born in the State of Louisiana may obtain a birth certificate from both our main office located in the Livingston Parish Courthouse as well as our Denham Springs location at 145 Del Orleans Ave., Bldg. 2, Suite E, Denham Springs, LA 70726.

DEATH CERTIFICATES
Certain family members may obtain a death certificate from both our main office and our Denham Springs location if the death occurred in the State of Louisiana after July 7, 2012. You will need to contact Vital Records in New Orleans at 504-593-5100 for any death certificate prior to this date.

APPLICATION FORM, IDENTIFICATION, AND FEES
A completed and signed application/request form is required along with the identification and fees listed on the form. These are received Monday through Friday between 8:00 a.m. – 3:30 p.m. at either location. (Our Denham Springs office closes from 12:00 – 1:00 p.m.) You can print the application/request form below or obtain one in our office.

If you need to alter or amend a birth or death certificate you must do so through Vital Records in New Orleans.

NOTE: For all out of state birth and death certificates, you must contact that state’s Vital Record’s office.

You may also obtain birth certificate information by contacting the Baton Rouge or the New Orleans office of Louisiana Vital Records at the addresses below or by visiting any of the Vital Records walk-in offices located around the state.
